First, I would ask the client if they feel as though they are getting anything out of treatment. If they say yes, continue on and if you feel no progress is being made, perhaps suggest a referral to another therapist, or someone higher up, such as a psychologist or psychiatrist. If they say no right away, refer them then. There is no shame in this.
